Almost half of the population of England now have a Summary Care Record. You can find out whether Summary Care Records have come to your area by looking at our interactive map.

When Summary Care Records are coming to your area you will receive an information pack. This pack will explain the changes to your records and help you choose whether you would like a Summary Care Record or not, before one is made for you.

If you choose to have a Summary Care Record you do not need to do anything. Sometime after the date mentioned in your letter, we will make one for you. 

Whatever you decide, you can change your mind at any time but you will need to tell your GP practice.
